In performing any exercise, there is a real chance of injury: no matter how “safe” or experienced you are.

In regard to the exercises depicted in the attached video I hereby release, indemnify, and discharge on behalf of myself and/or my spouse or children SPINE & SPORTS LLC, their officers, employees, and all other persons/organizations acting in any capacity on their behalf (which will be collectively referred to as "Spine & Sports"),:

I acknowledge that my participation in exercises depicted in the attached videos involve various movement forms, with known and/or unanticipated risks that could result in physical or emotional injury. I understand that such risks simply cannot be eliminated. My participation in any exercise is purely voluntary, and I elect to participate despite the risks. I understand that I must judge my own capabilities in regard to these activities and agree to act within the limits of my own body. I take full responsibility for my judgements and agree to act with safety as the number one priority.

I hereby voluntarily release and forever discharge, and agree to indemnify and hold harmless SPINE & SPORTS from any and all claims, demands or causes of action, which are in any way connected with my participation in exercise depicted. Furthermore, I agree to indemnify and hold SPINE & SPORTS harmless from any claim, cause of action or expense (including attorney fees) arising from or a result of any activity in which I am engaged.

By clicking on the I AGREE below, I acknowledge that if anyone is hurt during my participation in any exercise, I waive my right to maintain a lawsuit against SPINE & SPORTS on the basis of any claim from which I have released them herein.
